When does the insurance go into effect?
Coverage starts the first of the month following 60 days employment. (start date: 6/4/06, effective date: 9/1/06)
Are ‘significant others’ eligible for the health insurance?
No. Only legally married spouses and dependent children may apply for coverage.
Are ‘co-pays’ considered to be part of my deductible or maximum out of pocket expense?
No. Co-pays for office visits and prescription drugs will continue throughout the policy year for the Premier and Performance plans. The Blue Edge plan does not have co-pays.
What is the age limit for legally dependent children?
Unmarried children up to age 19, age 25 if they are a full time student. (You will be asked to provide proof of ‘student status’ on a yearly basis) Insurance will end on college graduation day or age of 25, whichever comes first.
What about a pre-existing condition waiting period?
We have a 1 year waiting period unless you can show that you have had previous health insurance coverage. Your waiting period is reduced by the length of time you had Credible Coverage. Pregnancy is not considered a pre-existing condition.
Are eye glasses covered?
No, but there is a list of providers from Blue Cross that will give you a generous discount if you show your Blue Cross card. Contact the number on the back of your card for a list of providers that offer this discount.
What type of plans are offered?
We offer 3 PPO plans with different deductible levels. We do not offer a HMO.
I’m on my spouse’s health insurance right now. If we lose that coverage, how soon would I be able to go on one of the Reedy plans?
Upon hire you must fill out an application and decline coverage stating you are covered on another policy. If that coverage is terminated, you and any dependents must apply for coverage within 31 days of the date of the previous insurance termination. The effective date of the new coverage is the date you apply for the new coverage.
What about wellness coverage?
All 3 of our plans have wellness coverage (routine physicals, immunizations, etc) paid at 100% after any applicable co-pay. You do not have to meet your deductible for wellness coverage.
Is there prescription drug coverage?
Yes. Depending upon which plan you choose, coverage will either be a co-pay or a percentage of the cost of the prescription.
